  • Applications need to be printed and mailed to the following address by September 12th, 2025:

  • Applicants will be notified via email that their application was received.

  • The grant provides financial support for one round of in vitro fertilization with preimplantation genetic testing of embryos:

    Medications associated with one egg retrieval

    One embryo egg retrieval

    Medications associated with one embryo transfer

    One embryo transfer

    PGT-M and PGT-A genetic testing of embryos

  • Yes, but the Guzi’s Hunt for the Cure Foundation does not cover costs associated with fertility issues or other health problems. See the question above about what the grant covers.

  • There is no fee to complete the application. However, selected applicants will be required to submit and pay for a background check.

  • No, applicants do not need to know their status nor complete testing. Applicants are required to provide proof of HD status in their immediate family (doctor’s note, lab test result, etc.)

  • Yes, applicants selected for the grant are required to meet with a financial coordinator at their selected fertility clinic. The goal is to ensure there is complete understanding about what the grant covers and what applicants will be expected to cover.

  • Yes, only residents of Wisconsin may apply at this time.

  • Yes, only couples/partners of Wisconsin may apply at this time. The Guzi’s Hunt for the Cure Foundation does not cover the cost of egg/sperm donors or surrogates.
